Breast cancer surgery has moved well beyond simple tumour removal — today’s approach combines complete oncological clearance with careful planning around shape, symmetry, and appearance from the very outset. Whether breast conservation or mastectomy is the right path depends on tumour size, location, and biology, but in either case, reconstructive and oncoplastic techniques are considered as part of the same surgical plan, not an afterthought.

Treatment begins with detailed imaging and biopsy to confirm diagnosis, tumour size, and extent, alongside assessment of lymph node involvement. Where breast conservation is appropriate, oncoplastic techniques reshape the breast at the time of tumour removal, maintaining natural contour. Where mastectomy is needed, skin-sparing or nipple-sparing approaches are used wherever oncologically safe, paired with immediate reconstruction using implants or the patient’s own tissue, restoring breast form in the same operation. Sentinel lymph node biopsy, often guided by ICG fluorescence mapping, provides accurate axillary staging while minimising the extent of lymph node removal.

Because breast cancer treatment often involves chemotherapy, radiation, or hormonal therapy alongside surgery, every case is discussed within a multidisciplinary tumour board — breast surgical oncology, plastic and reconstructive surgery, medical oncology, and radiation oncology working together to shape a plan that treats the cancer while protecting appearance and quality of life.

In most cases, yes. Immediate reconstruction using implants or your own tissue is planned as part of the same operation as mastectomy, though a staged approach may be recommended in select cases.

Most patients require sentinel lymph node biopsy to assess for cancer spread. More extensive axillary dissection is only needed if the sentinel nodes show significant involvement.

This depends on tumour type, stage, hormone receptor status, and lymph node involvement, and is determined by the multidisciplinary tumour board as part of your complete treatment plan.

In appropriately selected cases, nipple-sparing mastectomy may be possible, supporting more natural-looking reconstruction. This is assessed individually based on tumour location relative to the nipple.
